Patient Stories

What our patients say

★★★★★

“From the very first session, Dr. Boaz took a thoughtful, individualized approach. He didn’t just treat the symptom; he addressed the root cause of the problem… After about five sessions, my shoulder feels completely healed. I was able to shoulder press pain-free for the first time in six months… If you train hard and want a physical therapist who truly understands performance, movement, and long term health, Dr. Boaz is the person to see.”

Chad S. · Yelp review
★★★★★

“Boaz worked on my knee for a few months and got me back to biking, skiing, and light jogging… I had gone to 2 other PT offices prior to seeing him and they couldn’t get me to the place he got me. He had different technology: laser therapy, shock therapy, radio frequency therapy, that I believe healed my knee and got me strong again. I highly recommend him!”

Pamela H. · Yelp review
★★★★★

“As a competitive athlete, it’s refreshing to work with a PT that not only respects what you do, but supports you in wanting to get back to 100% healthy and doing whatever sport or activity you love… It was apparent how much he truly cared about helping me recover… I have never worked with another PT who was more ‘in my corner’ than Dr. B. He’s extremely smart, professional and an absolute pleasure to work with; I can’t recommend him enough!!”

April G. · Yelp review
★★★★★

“14 months of pain brought suddenly to an end… The doctor listened to my story, and they began treatment. I didn’t feel for a moment rushed. I never felt for a moment that their time was more valuable than my time. I was treated with dignity and respect… I was dubious about all of this, but the results speak for themselves… See you on the trails.”

About Us

Physical therapy the way it should be practiced

Founded in 2012, Aspire Health & Wellness was built on a simple conviction: patients heal faster with a full hour of undivided, one-on-one treatment from a Doctor of Physical Therapy and access to the best tools in the field.

Meet Dr. Boaz Ritblatt, PT, DPT, CSCS, Cert. ART

Doctor of Physical Therapy

Dr. Ritblatt's path to physical therapy began the way many of his patients' journeys begin: with an injury. In 2006, he was in a motorcycle accident. At the time, he was on his way to a career in law. Going through his own rehabilitation changed all of that. Sitting on the other side of the treatment table and doing the hard daily work of recovery, he found what he was truly meant to do with his life, and he never looked back.

That's still the foundation of how he practices today. Because he's been the patient, he doesn't just sympathize with what you're going through. He gets it because he's been there. He knows how frustrating a slow week can feel, what it's like to wonder whether you'll get back to the things you love, and how much it matters to have someone genuinely in your corner. That's the kind of care he set out to give every person who walks through the door.

Dr. Ritblatt earned his Doctor of Physical Therapy degree from the University of Southern California and has spent his career refining a whole-person approach to orthopedic and sports rehabilitation. He is a Certified Strength and Conditioning Specialist (CSCS) and a certified provider of Active Release Techniques® (ART®). His clinical experience spans professional athletes, Ironman competitors, and Division I collegiate athletes, as well as post-surgical patients.

Off the Clock

Dr. Ritblatt is married to the love of his life, and together they are raising two wonderful children: a 10-year-old daughter and an 8-year-old son. First and foremost, he is a devoted husband and father, and family is at the center of everything he does. He treasures their time together, especially traveling and making new memories as a family.

That is what drives him, and it's exactly what he wants to help every patient get back to: the people, activities, and experiences that make life meaningful.

He also lives the active life he helps his patients reclaim. An accomplished endurance athlete, he has completed a 70.3 Ironman and a 50-mile ultramarathon and stays active through CrossFit, HYROX, jiu-jitsu, skiing, golf, and hiking. Because he trains and competes himself, he understands firsthand what it takes to recover from an injury and get back to the activities you love.

Our Philosophy: Treat the Person, Not the Diagnosis

Where you feel pain isn't always where the problem begins. That's why every Aspire treatment plan starts with a comprehensive movement analysis to identify what's contributing to the problem, then addresses it from multiple angles with skilled manual therapy, corrective exercise, and advanced technology like PiezoWave2 acoustic wave therapy and Class IV deep tissue laser.

The goal isn't simply to chase symptoms. It's to understand what's driving them and help you move, perform, and feel better.

Why We Stay Independent and One-on-One

Traditional insurance-based physical therapy often means shorter appointments, multiple patients being treated at the same time, and portions of care delegated to aides or assistants. Aspire was intentionally built differently.

As an out-of-network practice, we have the freedom to focus on what we believe matters most: a full hour, one-on-one with your Doctor of Physical Therapy. No rotating between patients. No being handed off to an aide. Just focused, individualized treatment built around you and your goals.

That independence gives us the time to look beyond where it hurts, treat the whole person, and make every visit count.

Signature Approach

Synergistic Care & Regenerative Orthopedic Program

Two Specialists. Fourteen+ Years of Collaboration.

Regenerative medicine starts the healing. Rehabilitation finishes the recovery.

For injuries that haven't responded to traditional care, rehabilitation and regenerative medicine shouldn't happen separately. Through more than 14 years of collaboration, Dr. Boaz Ritblatt, DPT (Doctor of Physical Therapy) and Dr. Bronner Handwerger, NMD have developed a coordinated approach that combines regenerative medicine with advanced rehabilitation to help patients recover more completely.

Why “Synergistic Care”?

Most patients are left to coordinate their own care. They receive an injection from one provider, physical therapy from another, and hope everyone stays on the same page.

Our approach is different.

For more than 14 years, Dr. Ritblatt and Dr. Handwerger, NMD have worked together to understand exactly how regenerative medicine and rehabilitation complement one another. Every recommendation is made with the next phase of treatment in mind. Instead of two providers working independently, your care is designed as one integrated strategy from the very beginning.

This is what we call Synergistic Care. Every phase of treatment supports the next, allowing healing, movement, strength, and function to progress together rather than in separate pieces.

Why Combining Regenerative Medicine and Rehabilitation Works Better

Regenerative medicine and rehabilitation solve different parts of the same problem.

Dr. Handwerger, NMD helps stimulate the body's natural healing response in damaged tendons, ligaments, joints, and other orthopedic tissues through regenerative medicine. Dr. Ritblatt restores mobility, rebuilds strength, corrects faulty movement patterns, and safely guides that healing tissue back to full function.

One prepares the tissue to heal.
The other teaches your body how to use it again.

Neither replaces the other. Together, they create a coordinated recovery strategy that addresses both tissue healing and long-term function, helping you return to work, sport, and the activities you love.

What a Synergistic Treatment Plan Looks Like

Every patient is different, so no two treatment plans are exactly alike.

A patient with chronic Achilles tendinopathy, for example, may receive regenerative treatment from Dr. Bronner Handwerger, NMD to stimulate healing within the tendon. As that healing progresses, Dr. Ritblatt builds rehabilitation around each stage of recovery using hands-on manual therapy, PiezoWave2 focused shockwave therapy, Fascial Manipulation®, ART®, IASTM, 45W Class IV deep tissue laser, corrective exercise, and movement retraining.

Because the two doctors have worked together for more than 14 years, treatment doesn't happen in isolation. Rehabilitation evolves alongside the body's healing process, with each phase intentionally supporting the next.

That's what makes this program different. It isn't simply regenerative medicine plus physical therapy. It's two specialists with complementary expertise following one coordinated treatment plan.

A 14-Year Clinical Partnership

This is more than a referral relationship.

Dr. Boaz Ritblatt, DPT (Doctor of Physical Therapy) and Dr. Bronner Handwerger, NMD have worked side by side for more than 14 years, refining how rehabilitation and regenerative medicine work together. You're not being referred across town to someone you've never met. You're cared for by two providers who have spent well over a decade collaborating, communicating, and developing an integrated approach to orthopedic recovery.

That continuity means your rehabilitation is designed around your regenerative treatment, and your regenerative treatment is planned with your rehabilitation in mind.

How the Program Works

The timing is intentional.

After a comprehensive evaluation, Dr. Ritblatt and Dr. Handwerger, NMD determine whether regenerative medicine should be part of your treatment plan.

When appropriate, Dr. Handwerger, NMD initiates the regenerative phase of treatment. Dr. Ritblatt then protects healing tissue early and progressively restores mobility, strength, and movement as your body recovers.

Throughout the process, both doctors remain aligned on your progress, allowing treatment to evolve as your recovery unfolds.

Our bundled treatment packages are designed to keep every phase of your care coordinated, from the initial evaluation through your return to work, sport, or everyday life.

What the Regenerative Side Includes

Through Dr. Bronner Handwerger, NMD and Integrative Health Solutions, appropriate candidates may receive:

  • Platelet-Rich Plasma (PRP): Uses a concentrated portion of your own blood to support the body's natural healing response in injured tendons, ligaments, muscles, and joints.
  • Prolotherapy: A regenerative injection therapy that stimulates the body's natural repair response in injured ligaments and tendons.
  • ACELL®: A regenerative extracellular matrix therapy used for select orthopedic conditions to support tissue repair.

The most appropriate treatment depends on your diagnosis, goals, and clinical evaluation.

About Dr. Bronner Handwerger, NMD

Dr. Bronner Handwerger, NMD is a licensed naturopathic medical doctor specializing in regenerative and integrative medicine. Over his career, he has treated professional NFL and Olympic athletes, Special Operations personnel, and active individuals seeking alternatives to surgery.

He earned his medical doctorate from the University of Bridgeport College of Naturopathic Medicine, where he later served as an Assistant Professor of Clinical Medicine. For more than two decades, he has helped patients recover from acute and chronic orthopedic injuries using advanced regenerative therapies.

Who It's For

This program may be appropriate for patients experiencing:

  • Chronic tendon injuries, including tennis elbow, Achilles tendinopathy, patellar tendinopathy, and rotator cuff injuries
  • Ligament sprains and instability
  • Muscle tears
  • Joint pain and osteoarthritis
  • Stress fractures and delayed healing
  • Persistent pain that has not improved with conventional physical therapy alone

It is especially valuable for people hoping to avoid surgery, reduce reliance on long-term medication, or move beyond a plateau after conventional treatment.

Whether you're an athlete trying to return to competition or someone simply wanting to get back to an active, pain-free life, the Synergistic Care & Regenerative Orthopedic Program is designed to help your body heal, move, and perform at its highest level.

Precision Focused Shockwave Therapy

PiezoWave² Focused Shockwave Therapy

Precision acoustic-wave technology that reaches deep, stubborn pain with pinpoint accuracy, without needles, medication, or downtime.

The PiezoWave² is an advanced focused extracorporeal shockwave therapy (ESWT) system that uses piezoelectric technology to deliver precisely focused mechanical acoustic waves deep into injured muscles, tendons, fascia, and connective tissue.

Unlike many radial “shockwave” devices that disperse energy outward near the surface, the PiezoWave² concentrates its therapeutic energy at a specific treatment depth. That means Dr. Ritblatt (Doctor of Physical Therapy) can target the true source of pain while minimizing unnecessary treatment of surrounding tissue. The result is a treatment that is precise, efficient, and comfortable while reaching structures that hands alone often cannot.

How It Helps You Heal

Focused shockwave therapy works by delivering mechanical energy into damaged tissue. Those acoustic waves create controlled mechanical stimulation that encourages the body's natural healing response.

Research has shown focused shockwave therapy can help:

  • Stimulate blood vessel formation and improve local circulation
  • Encourage the body's normal tissue repair processes
  • Break the cycle of chronic tendon degeneration
  • Reduce pain by altering pain signaling and decreasing hypersensitive trigger points
  • Improve tissue mobility by addressing fascial restrictions and chronic adhesions
  • Promote collagen remodeling within injured tendons and connective tissue
  • Restore movement while reducing muscle guarding and protective spasm

Many patients describe the treatment as finally reaching “the exact spot” that's been causing pain all along. That's because the PiezoWave² focuses energy directly at the source of dysfunction instead of simply treating the surface.

Even better, treatment requires no injections, no medication, and virtually no downtime, allowing most patients to return to normal daily activities immediately afterward.

Why Shockwave Works

Shockwave therapy doesn't just mask pain. It triggers the biological processes that produce genuine tissue repair.

New Blood Supply

Focused acoustic waves prompt the body to grow new blood vessels in the treated area. Chronic tendon injuries are often starved of blood flow, and that fresh supply delivers the oxygen and growth factors needed to restart repair that had stalled.

Collagen Remodeling

The waves stimulate the cells that build collagen, encouraging new, well-organized fibers. In damaged tendon where the collagen has become disorganized and weak, that remodeling helps rebuild the tissue's strength and its ability to handle load.

Calcification Breakdown

PiezoWave²'s focused piezoelectric energy is especially effective at fragmenting calcium deposits inside tendons. Once the deposit is broken apart, the body absorbs and clears the debris, removing a common source of pain without needles or surgery.

Pain Desensitization

Shockwave lowers levels of substance P, a chemical that makes pain receptors more sensitive, within the treated tissue. The result is both immediate relief and a longer-term calming of nerves that have become chronically overreactive.

Growth Factor Release

The mechanical stimulus prompts your body to release its own repair signals, including the growth factors that recruit healing cells to the injury. This amplifies your natural regenerative response right where it's needed most.

Trigger Point Release

Focused waves reach the deep trigger points that keep a muscle knotted and guarding, interrupting the localized energy crisis that sustains them. As circulation and normal muscle function return, the referred pain those trigger points create eases as well.

Why We Chose PiezoWave²

Not all shockwave therapy is the same.

Many clinics use radial pressure-wave devices, which spread energy outward from the applicator and primarily treat more superficial tissue. The PiezoWave² uses focused piezoelectric shockwave technology, allowing Dr. Ritblatt to precisely control both the treatment depth and focal point.

That precision makes a meaningful difference when treating deep trigger points, chronic tendinopathies, and injuries involving structures well beneath the skin.

The PiezoWave²'s direct-focusing technology also allows treatment intensity to be adjusted independently of treatment depth, providing highly targeted care while minimizing discomfort during treatment.

What It's Used For

Focused shockwave therapy is commonly used to treat:

  • Plantar fasciitis
  • Achilles tendinopathy
  • Patellar tendinopathy (jumper's knee)
  • Tennis elbow and golfer's elbow
  • Rotator cuff tendinopathy
  • Calcific shoulder tendinopathy
  • Hamstring injuries
  • Hip and gluteal tendinopathy
  • Myofascial pain syndrome
  • Chronic trigger points
  • Scar tissue and fascial restrictions
  • Muscle strains
  • Running and overuse injuries

Shockwave therapy is especially valuable for chronic conditions that have failed to fully respond to rest, medication, massage, injections, or traditional rehabilitation alone.

Why It's Different at Aspire

The PiezoWave² isn't a standalone treatment.

When indicated, Dr. Ritblatt combines focused shockwave therapy with skilled manual therapy, Fascial Manipulation®, ART®, IASTM, 45W Deep Tissue Laser Therapy, corrective exercise, and other advanced treatment techniques during the same one-hour session.

By integrating multiple complementary therapies, the effects of shockwave treatment are reinforced immediately through improved mobility, restored movement, and retraining of healthy movement patterns. It's one more piece of Aspire's Synergistic Care philosophy, where every treatment builds on the one before it.

Feel the Difference in One Session

PiezoWave² isn't an add-on or an upsell. When Dr. Ritblatt believes focused shockwave therapy will improve your outcome, it's simply incorporated into your treatment plan as part of your one-hour, one-on-one session.

High-Power Laser Therapy

45W Deep Tissue Laser Therapy

High-powered Class IV laser therapy that delivers therapeutic light deep into muscles, joints, tendons, and nerves to reduce pain, calm inflammation, and stimulate your body's natural healing process.

Unlike low-level “cold” lasers, our 45-watt Class IV Deep Tissue Laser delivers high-intensity therapeutic energy to both superficial and deep tissues in just minutes. Through a process known as photobiomodulation, light energy is absorbed by your cells and converted into the fuel they need to repair damaged tissue, helping you recover naturally without medication, injections, or downtime.

Healing at the Speed of Light

Every cell in your body depends on energy to heal.

Our 45-watt Class IV Deep Tissue Laser delivers concentrated light energy deep into injured tissue, where specialized structures inside your cells called mitochondria absorb it. This process, known as photobiomodulation, stimulates production of adenosine triphosphate (ATP), the primary source of cellular energy.

With more energy available, your cells can perform the work of healing more efficiently.

Research has shown photobiomodulation can help:

  • Increase ATP (cellular energy) production
  • Improve blood flow and oxygen delivery
  • Support collagen production and tissue remodeling
  • Reduce inflammation and oxidative stress
  • Calm irritated nerve tissue
  • Decrease pain and muscle guarding
  • Accelerate recovery following injury or surgery

Rather than simply masking symptoms, 45W Deep Tissue Laser Therapy supports your body's own repair process at the cellular level.

Why 45 Watts Matters

Not all laser therapy is created equal.

Many clinics use low-level “cold” lasers measured in milliwatts. While those systems have their place, they deliver relatively small amounts of energy and primarily treat tissues near the surface.

Our 45-watt Class IV Deep Tissue Laser delivers significantly more therapeutic energy, allowing treatment to reach larger muscles, deeper joints, thick tendons, and structures several centimeters beneath the skin in treatment times of just a few minutes.

That means conditions involving the hips, shoulders, spine, gluteal muscles, hamstrings, Achilles tendon, and other deep orthopedic structures can be treated more efficiently.

Simply put, higher power allows more therapeutic energy to reach the tissue that needs it, making 45W Deep Tissue Laser Therapy especially effective for stubborn injuries and chronic pain.

Two Wavelengths. One Complete Treatment.

Our 45W Deep Tissue Laser combines 980 nm and 1064 nm wavelengths, allowing treatment of both superficial tissues and deep orthopedic structures during the same session.

980 nm: The Pain and Circulation Wavelength

The 980 nm wavelength is readily absorbed by water and hemoglobin, making it particularly effective in superficial muscle, fascia, and small blood vessels.

Its primary benefits include:

  • Increased local circulation
  • Vasodilation (improved blood flow)
  • Better oxygen and nutrient delivery
  • Removal of inflammatory byproducts
  • Reduced muscle tightness and stiffness
  • Rapid pain relief

Many patients notice improved mobility and less discomfort immediately after treatment.

1064 nm: The Deep Healing Wavelength

The 1064 nm wavelength penetrates much deeper because it is less readily absorbed by water and skin pigment.

That allows therapeutic light to reach:

  • Deep muscle tissue
  • Hip and shoulder joints
  • Spinal structures
  • Thick tendons
  • Ligaments
  • Peripheral nerves

At these depths, light stimulates mitochondrial activity, increasing ATP production while encouraging collagen synthesis, angiogenesis (formation of new blood vessels), tissue remodeling, and long-term healing.

This deeper penetration is what makes 45W Deep Tissue Laser Therapy especially valuable for chronic orthopedic conditions that have not responded to traditional treatment.

Why Using Both Wavelengths Matters

Most laser systems force a compromise.

Treat the surface well.
Or treat deep tissue well.

Our dual-wavelength system treats both during the same session.

While the 980 nm wavelength helps calm pain and inflammation closer to the surface, the 1064 nm wavelength stimulates cellular repair deep within muscles, tendons, joints, and connective tissue where chronic pain often begins.

The result is immediate pain relief combined with meaningful tissue healing beneath the surface.

Conditions Commonly Treated

45W Deep Tissue Laser Therapy is commonly used to treat:

  • Back pain
  • Neck pain
  • Shoulder pain
  • Hip pain
  • Knee pain
  • Arthritis and joint pain
  • Tendonitis and tendinopathy
  • Rotator cuff injuries
  • Tennis elbow
  • Golfer's elbow
  • Plantar fasciitis
  • Achilles tendinopathy
  • Bursitis
  • Muscle strains
  • Ligament injuries
  • Sciatica
  • Peripheral nerve irritation
  • Post-surgical recovery
  • Sports injuries
  • Running and overuse injuries
  • Chronic inflammation

Treatments are completely non-invasive. Most patients experience only a comfortable, soothing warmth during therapy, with no recovery time afterward.

Why It's Different at Aspire

At Aspire Health & Wellness, 45W Deep Tissue Laser Therapy is never a standalone treatment.

When appropriate, Dr. Ritblatt (Doctor of Physical Therapy) combines the 45W Deep Tissue Laser with PiezoWave² Focused Shockwave Therapy, skilled manual therapy, Fascial Manipulation®, ART®, IASTM, and individualized corrective exercise during the same one-hour session.

By integrating multiple complementary therapies, each treatment builds upon the one before it. This is the foundation of Aspire's Synergistic Care philosophy, helping restore movement, reduce pain, and support long-term healing rather than simply providing temporary relief.

Experience the Power of 45W Deep Tissue Laser Therapy

45W Deep Tissue Laser Therapy isn't an add-on or an upsell. When Dr. Ritblatt believes it will improve your outcome, it's incorporated into your personalized treatment plan as part of your one-hour, one-on-one session.

Advanced Soft Tissue Therapy

Active Release Techniques® (ART®) in San Diego

The gold-standard movement-based soft tissue treatment trusted by professional athletes worldwide, delivered by a certified ART® provider.

Active Release Techniques® (ART®) is a patented soft tissue treatment system designed to identify and treat disorders affecting muscles, tendons, ligaments, fascia, and nerves. Whether pain developed from repetitive motion, sports, exercise, poor posture, or an acute injury, ART® helps restore normal movement by addressing the scar tissue and adhesions that often limit recovery.

What Is ART®?

Healthy muscles and connective tissue are designed to glide smoothly over one another during movement.

When tissues become overloaded through repetitive strain, athletic training, surgery, or injury, your body lays down scar tissue as part of the healing process. Over time, those adhesions can bind neighboring tissues together, reducing mobility, limiting flexibility, weakening muscles, restricting circulation, and irritating nearby nerves.

That's where Active Release Techniques® is different.

Rather than simply massaging sore muscles, ART® identifies the specific tissue that has become restricted and treats the underlying cause of dysfunction using precise, movement-based protocols developed through decades of clinical research and refinement.

How ART® Works

Every ART® treatment combines two things:

  • Precise tension applied by the provider
  • Specific active movement performed by the patient

As you move through a carefully guided motion, Dr. Ritblatt (Doctor of Physical Therapy) applies targeted pressure to the affected tissue, helping restore normal movement between muscles, fascia, tendons, ligaments, and nerves.

Each treatment is based on one of more than 500 patented ART® protocols, with every protocol designed for a specific anatomical structure.

This isn't a massage.
It isn't generalized soft tissue work.

Every contact is intentional, every movement has a purpose, and every treatment is designed to restore healthy tissue mobility while reducing pain and improving function.

Why ART® Works

Scar tissue itself isn't the problem.

The problem occurs when scar tissue prevents tissues from sliding normally during movement.

That restriction can lead to:

  • Chronic muscle tightness
  • Loss of flexibility
  • Reduced strength
  • Pain during movement
  • Nerve irritation or entrapment
  • Poor movement mechanics
  • Recurring injuries

By restoring normal tissue mobility, ART® helps reduce mechanical stress on muscles, tendons, and nerves while allowing your body to move the way it was designed.

For many patients, improved movement is noticeable immediately after treatment.

ART® has become a staple of professional sports medicine because it allows athletes to restore movement quickly while continuing to perform at a high level.

Why It's Different at Aspire

ART® is rarely performed in isolation.

When appropriate, Dr. Ritblatt combines Active Release Techniques® with PiezoWave² Focused Shockwave Therapy, 45W Deep Tissue Laser Therapy, Fascial Manipulation®, IASTM, joint mobilization, and corrective exercise during the same one-hour treatment session.

That combination is part of Aspire's Synergistic Care philosophy, where every treatment builds upon the one before it to improve mobility, reduce pain, and restore long-term movement rather than simply relieving symptoms.

Experience the Difference

Active Release Techniques® isn't an add-on or an upsell. When Dr. Ritblatt believes ART® is the best treatment for your condition, it becomes part of your personalized one-hour treatment session.

Advanced Manual Therapy

IASTM (Instrument-Assisted Soft Tissue Mobilization)

Precision stainless-steel instruments help identify and treat scar tissue, fascial restrictions, and chronic soft tissue dysfunction that can limit movement and contribute to pain.

Instrument-Assisted Soft Tissue Mobilization (IASTM) is an advanced manual therapy technique that uses specially designed stainless-steel instruments to evaluate and treat muscles, tendons, ligaments, fascia, and scar tissue. Dr. Ritblatt completed Graston Technique® training in 2014 and now uses HawkGrips® instruments to provide precise, evidence-informed IASTM treatment as part of your one-on-one rehabilitation program.

Why Use Instruments Instead of Hands?

Your hands can feel a lot.
Specialized IASTM instruments can often detect even more.

The beveled stainless-steel edges glide across the skin, allowing Dr. Ritblatt (Doctor of Physical Therapy) to identify areas of tissue restriction, adhesions, fibrosis, and abnormal fascial tension that may be difficult to appreciate with hands alone.

Once those restrictions are located, the same instruments deliver carefully controlled treatment designed to restore normal tissue mobility while stimulating the body's natural healing response.

The result is greater precision, improved feedback, and highly targeted treatment.

How IASTM Helps You Heal

Healthy muscles, fascia, tendons, and ligaments should move freely against one another.

Following injury, surgery, repetitive overuse, or chronic inflammation, scar tissue can develop in a disorganized pattern. Those adhesions may restrict movement, reduce flexibility, decrease strength, alter movement mechanics, and contribute to ongoing pain.

IASTM helps by:

  • Breaking down scar tissue and soft tissue adhesions
  • Improving fascial mobility
  • Stimulating local circulation
  • Encouraging collagen remodeling
  • Supporting the body's normal healing response
  • Improving flexibility and range of motion
  • Reducing pain and movement restrictions

For many patients, improved movement is noticeable immediately after treatment.

When combined with corrective exercise, newly restored mobility can be reinforced through healthier movement patterns, helping reduce the likelihood of recurring dysfunction.

Conditions Commonly Treated

IASTM is commonly used to treat:

  • Chronic tendinopathy
  • Tennis elbow
  • Golfer's elbow
  • Achilles tendinopathy
  • Rotator cuff injuries
  • Plantar fasciitis
  • IT band syndrome
  • Patellar tendinopathy
  • Hamstring strains
  • Calf strains
  • Hip pain
  • Neck pain
  • Back pain
  • Post-surgical scar tissue
  • Myofascial restrictions
  • Muscle strains that never fully healed
  • Running injuries
  • Sports injuries
  • Chronic movement restrictions following old injuries

IASTM is particularly valuable for chronic soft tissue injuries that have plateaued with stretching, massage, or exercise alone.

Why It's Different at Aspire

At Aspire Health & Wellness, IASTM is never used by itself.

When appropriate, Dr. Ritblatt combines instrument-assisted soft tissue mobilization with Active Release Techniques® (ART®), Fascial Manipulation®, PiezoWave² Focused Shockwave Therapy, 45W Deep Tissue Laser Therapy, joint mobilization, and corrective exercise during the same one-hour treatment session.

That combination is part of Aspire's Synergistic Care philosophy, where every treatment builds upon the one before it to restore movement, reduce pain, and support long-term recovery.

Rather than simply breaking up tissue restrictions, the goal is to immediately restore healthy movement while teaching your body how to maintain those improvements.

Precision Makes the Difference

IASTM isn't an add-on or an upsell. When Dr. Ritblatt believes instrument-assisted soft tissue mobilization will improve your outcome, it becomes part of your personalized one-hour treatment session.

Advanced Fascial Therapy

Fascial Manipulation® (Stecco Method) | San Diego

A research-driven manual therapy that treats the body's fascial system as a whole, helping resolve pain and movement restrictions that often persist despite traditional treatment. Dr. Ritblatt completed a three-day Fascial Manipulation® course taught by Dr. Antonio Stecco, MD, PhD.

Understanding Fascia: Your Body's Connective Network

Fascia is a continuous three-dimensional web of connective tissue that surrounds, supports, and connects every muscle, tendon, ligament, nerve, joint, blood vessel, and organ throughout the body.

Rather than functioning as isolated parts, your body's tissues work together through this interconnected fascial network. Because of that connection, a restriction in one area can alter movement and force transmission somewhere entirely different.

That's why shoulder pain may actually begin in the upper back, knee pain may originate at the hip, or neck pain may be influenced by restrictions elsewhere in the body.

Treating only where it hurts doesn't always solve the problem.

What Causes Pain? Understanding Fascial Densification

Healthy fascial layers are designed to glide smoothly over one another during movement.

Between those layers is hyaluronan (HA), a naturally occurring substance that acts as a lubricant, allowing tissues to slide freely.

Following injury, repetitive overuse, surgery, inflammation, or prolonged immobility, that hyaluronan can become more viscous and adhesive. Instead of allowing smooth movement, the fascial layers begin sticking together, creating what the Stecco Method calls densification.

Those densified areas reduce tissue mobility, alter force transmission, restrict movement, and can contribute to pain both locally and in areas far removed from the original restriction.

One of the primary goals of Fascial Manipulation® is identifying and treating these specific areas of densification rather than simply treating where symptoms are felt.

A Systematic, Evidence-Based Approach

Fascial Manipulation®, developed by Italian physical therapist Luigi Stecco and advanced through decades of anatomical and clinical research by Dr. Carla Stecco and Dr. Antonio Stecco, is a structured clinical method, not generalized bodywork or massage.

Every treatment begins with a systematic evaluation.

Rather than asking only, “Where does it hurt?” the Stecco Method asks:

  • How does your body move?
  • Which movement patterns are restricted?
  • Where is force no longer transferring efficiently?
  • Which fascial points are responsible for those movement changes?

Using your history, movement assessment, and detailed palpation, Dr. Ritblatt (Doctor of Physical Therapy) identifies the specific fascial points contributing to your symptoms.

Treatment then focuses on those precise locations using controlled, deep manual friction designed to restore normal fascial glide and improve movement throughout the kinetic chain.

What a Fascial Manipulation® Session Looks Like

Every treatment follows a structured clinical process.

Dr. Ritblatt begins by reviewing your injury history and evaluating movement across multiple planes. He then palpates specific fascial points to identify areas of densification that correspond to your movement dysfunction.

Treatment targets those identified points rather than simply treating the painful area itself.

Because Fascial Manipulation® stimulates a controlled biological response within the fascial system, many patients notice continued improvement over the following 24 to 48 hours as tissue mobility continues to improve.

The goal isn't simply temporary pain relief. It's restoring how your body moves as an integrated system.

Why Dr. Ritblatt Chose the Stecco Method

Dr. Ritblatt completed a three-day Fascial Manipulation® course in San Francisco taught by Dr. Antonio Stecco, MD, PhD, one of the world's leading researchers and educators in fascial science.

Dr. Antonio Stecco serves as a Research Assistant Professor in the Department of Rehabilitation Medicine at NYU Grossman School of Medicine and has authored more than 50 peer-reviewed scientific publications investigating fascia, biomechanics, and musculoskeletal pain.

Learning the Stecco Method directly from one of its foremost experts gave Dr. Ritblatt advanced training in one of the most research-driven fascial treatment systems available today.

Conditions Commonly Treated

Fascial Manipulation® is especially valuable for patients experiencing:

  • Chronic neck pain
  • Back pain
  • Shoulder pain
  • Hip pain
  • Knee pain
  • Persistent movement restrictions
  • Recurrent sports injuries
  • Post-surgical stiffness
  • Fascial adhesions
  • Myofascial pain
  • Multi-region pain patterns
  • Chronic overuse injuries
  • Running injuries
  • Movement dysfunction without a clear structural diagnosis

It is particularly useful when pain continues despite traditional physical therapy, massage, chiropractic care, injections, or other conservative treatments.

Why It's Different at Aspire

Fascial Manipulation® is rarely used as a standalone treatment.

When appropriate, Dr. Ritblatt combines the Stecco Method with Active Release Techniques® (ART®), Instrument-Assisted Soft Tissue Mobilization (IASTM), PiezoWave² Focused Shockwave Therapy, 45W Deep Tissue Laser Therapy, joint mobilization, and individualized corrective exercise during the same one-hour treatment session.

That combination is part of Aspire's Synergistic Care philosophy, where every treatment builds upon the one before it to restore movement, reduce pain, and support long-term recovery.

Rather than simply chasing symptoms, the goal is to identify the true driver of dysfunction and restore healthy movement throughout the entire kinetic chain.

Movement-Based Cupping Therapy

Myofascial Decompression (Cupping Therapy) in San Diego

Modern, movement-based cupping that lifts restricted tissue, restores healthy fascial movement, and improves mobility through a research-informed rehabilitation approach.

Unlike traditional massage, which compresses tissue, Myofascial Decompression (MFD) uses negative pressure to gently lift the skin, fascia, and underlying muscle away from deeper structures. This decompression helps restore normal tissue glide, improve circulation, reduce movement restrictions, and prepare the body for healthier movement.

At Aspire Health & Wellness, cupping isn't a spa treatment. It's a clinical rehabilitation technique integrated into your one-on-one treatment session when it's the right tool for your condition.

Why Decompression Matters

Most manual therapy works by applying pressure.

Myofascial Decompression works by doing the opposite.

Using specialized medical-grade cups, Dr. Ritblatt (Doctor of Physical Therapy) creates gentle negative pressure that lifts restricted tissue instead of compressing it.

This decompression may help:

  • Restore normal fascial glide
  • Improve local circulation
  • Reduce tissue stiffness
  • Improve tissue hydration
  • Decrease muscle guarding
  • Improve mobility
  • Reduce discomfort associated with chronic soft tissue restriction

By separating tissue layers that have become restricted, Myofascial Decompression allows muscles and fascia to move more freely while supporting the body's natural healing process.

Movement Is What Makes It Different

Traditional cupping is often passive.

Our approach is active.

While the cups remain in place, Dr. Ritblatt guides you through specific movement patterns designed to restore mobility while the tissues are decompressed.

Moving during treatment helps your nervous system learn healthier movement patterns immediately, making the treatment more functional than simply lying still with cups attached.

This movement-based approach is one reason Myofascial Decompression has become increasingly popular among physical therapists, professional sports medicine teams, and elite athletes.

How Myofascial Decompression Helps You Heal

Negative pressure created by the cups produces controlled mechanical lifting within the soft tissues.

This may help:

  • Improve blood flow to the treated area
  • Reduce fascial restrictions
  • Improve tissue mobility
  • Restore normal movement between tissue layers
  • Improve flexibility and range of motion
  • Reduce muscular tension
  • Improve recovery following strenuous activity
  • Decrease post-exercise soreness
  • Improve movement quality

Many patients notice improved mobility immediately following treatment, particularly when Myofascial Decompression is combined with manual therapy and corrective exercise.

The circular marks that sometimes appear afterward are not bruises. They result from temporary changes in superficial circulation and typically fade over several days.

Conditions Commonly Treated

Myofascial Decompression (Cupping) is commonly used to treat:

  • Neck pain
  • Back pain
  • Shoulder restrictions
  • Hip tightness
  • Chronic muscle tightness
  • Scar tissue restrictions
  • Myofascial pain syndrome
  • Running injuries
  • Sports injuries
  • Post-workout recovery
  • Plantar fasciitis
  • Rotator cuff injuries
  • Thoracic mobility restrictions
  • Persistent movement limitations

It is especially valuable for patients whose tissues remain tight despite stretching, massage, or traditional rehabilitation.

Why It's Different at Aspire

At Aspire, Myofascial Decompression is never a standalone service.

When appropriate, Dr. Ritblatt combines Myofascial Decompression with Active Release Techniques® (ART®), Fascial Manipulation®, Instrument-Assisted Soft Tissue Mobilization (IASTM), PiezoWave² Focused Shockwave Therapy, 45W Deep Tissue Laser Therapy, skilled manual therapy, and corrective exercise during the same one-hour treatment session.

That combination is part of Aspire's Synergistic Care philosophy, where every treatment builds upon the one before it to improve mobility, reduce pain, and support long-term recovery.

Rather than simply helping you feel better temporarily, the goal is to restore healthier movement that lasts.

Frequently Asked Questions

Everything you need to know before your first visit.

Why is Aspire different from traditional physical therapy?

Most physical therapy clinics schedule multiple patients at the same time, rely on aides or assistants for portions of treatment, and separate manual therapy, exercise, and advanced modalities into different visits, or don't offer them at all.

Aspire Health & Wellness was intentionally built differently.

Every appointment is a full 60 minutes, one-on-one with Dr. Boaz Ritblatt, Doctor of Physical Therapy. Your care is never handed off to an aide or assistant. Instead, Dr. Ritblatt performs your evaluation, delivers every treatment personally, and continually adjusts your plan based on how your body responds.

When appropriate, your session may combine advanced manual therapy, PiezoWave² Focused Shockwave Therapy, 45W Deep Tissue Laser Therapy, Active Release Techniques® (ART®), Fascial Manipulation® (Stecco Method), Instrument-Assisted Soft Tissue Mobilization (IASTM), Myofascial Decompression (Cupping), joint mobilization, and corrective exercise, all during the same visit.

This integrated approach is called Synergistic Care. Rather than relying on a single treatment, multiple complementary techniques are thoughtfully combined so each builds on the one before it. The result is highly individualized care designed to identify the true source of your pain, restore healthy movement, and help you return to the activities you love.

Do you take insurance?

Aspire Health & Wellness is an out-of-network, cash-based physical therapy practice by design.

That allows every visit to be a full hour, one-on-one with Dr. Boaz Ritblatt, Doctor of Physical Therapy, instead of shorter appointments split between multiple patients or delegated to aides and assistants.

We'll gladly provide a detailed superbill that you can submit to your insurance company for possible out-of-network reimbursement. Many patients find that because they often recover in fewer visits, their overall cost is comparable to, or even less than, traditional in-network care.

Why don't you accept insurance?

Insurance companies often dictate how long appointments can be, how many visits are authorized, and which treatments are covered.

Remaining independent allows us to focus on what you need instead of what an insurance company approves.

Every treatment plan is built around your goals, not insurance limitations.

Do I need a doctor's referral to start physical therapy?

In most cases, no. California's direct access law allows you to begin physical therapy without a physician referral.

If your condition requires a referral for continued care or for insurance reimbursement, we'll let you know and help guide you through the process.

What should I expect at my first appointment?

Your first visit includes much more than an evaluation.

Dr. Ritblatt will review your medical and injury history, perform a comprehensive movement assessment, evaluate the joints, muscles, nerves, and connective tissue involved, and identify the underlying cause of your symptoms.

You'll also receive treatment during your first visit whenever appropriate.

Before you leave, you'll understand:

  • What’s causing your pain
  • Why it developed
  • How we’ll treat it
  • What to expect during recovery
  • Approximately how many visits you’ll need
Will I receive treatment during my first visit?

Yes.

Whenever appropriate, treatment begins during your initial evaluation. Depending on your condition, that may include manual therapy, corrective exercise, PiezoWave² Focused Shockwave Therapy, 45W Deep Tissue Laser Therapy, Active Release Techniques® (ART®), Fascial Manipulation®, Instrument-Assisted Soft Tissue Mobilization (IASTM), Myofascial Decompression (Cupping), or other treatment techniques.

Every treatment plan is customized to your specific injury and goals.

How long is each appointment?

Every appointment is a full 60 minutes, one-on-one with Dr. Boaz Ritblatt, Doctor of Physical Therapy.

You'll never be handed off to an aide, technician, or assistant.

How many visits will I need?

Every injury is different.

Your treatment plan depends on the nature of your condition, your goals, and how your body responds to treatment.

After your evaluation, Dr. Ritblatt will provide an honest estimate of the expected number of visits and update that plan as your recovery progresses.

The goal is always to help you recover as efficiently as possible, not to keep you in treatment longer than necessary.

Do you treat athletes only?

Not at all.

While Dr. Ritblatt has treated professional athletes, Ironman competitors, and Division I collegiate athletes, the majority of patients simply want to move better, stay active, and return to everyday life without pain.

Whether your goal is returning to sport, playing with your children, gardening, hiking, or simply getting through the workday comfortably, your treatment plan is built around your goals.
